Health Insurance and Fresh Food Program
WellSense Care Alliance and Mill City Grows
Mill City Grows is working with WellSense Care Alliance to give fresh fruits and vegetables to people who qualify.
This is part of a program through MassHealth that helps people who need more healthy food, called the Health-Related Social Needs Services (HRSN) Program.
If you qualify, you could get a $125 gift card to buy food at the Mill City Grows Mobile Farmers Market which runs all year long. This card refills every month for 6 months, as long as you still qualify.
Who can join this program?
To be a part of this program, these things must be true:
1. You must reside in the Lowell area.
2. You have health insurance through the WellSense Care Alliance ACO. If you have different insurance, you may still be able to get help — please ask your healthcare provider.
3. You or someone in your home can cook the fresh food you get.
4. You have a hard time getting enough food. This means you sometimes skip meals or don’t have enough money for groceries. We’ll ask you 6 short questions from the USDA to check this.
5. You have a health issue like:
- Anxiety, depression, ADHD (or another behavioral health diagnosis)
- Stomach conditions, nervous system problems, obesity (or another physical health diagnosis)
- You are pregnant, need help with daily tasks, or have gone to the emergency room 2 or more times in the last 6 months
Please note: Not every health problem will qualify you for the program.
