WellSense Care Alliance and Mill City Grows

Mill City Grows has partnered with WellSense Care Alliance to provide fresh local produce to those who qualify through the MassHealth Health Related Social Needs (HRSN) Program.

If you meet the qualifications, you could receive:

  • Up to 6 months of the Mill City Grows Farm Share delivery service
    or
  • a $125 gift card to the Mill City Grows Mobile Market. The card will reload every month for 6 months for as long as you remain eligible. 

 

Health Insurance Partnership Program Eligibility:

1. You must reside in the Lowell area.

2. You must have WellSense Care Alliance.
These services are available to eligible MassHealth ACO members only. If you have a different ACO, you may qualify for nutrition services from a different HRSN nutrition provider. Please contact your healthcare provider. 

3. You or someone in your household must be able to cook the fresh food you receive.

4. You must be experiencing very low food security.
Very low food security is defined as: “Food intake of household members is reduced and their normal eating patterns are disrupted because the household lacks money and other resources for food.”

We will determine your food security level by asking you a 6-question USDA survey required by MassHealth.

5. You must have a qualifying condition (health needs based criteria), such as:

  • Behavioral health needs, such as anxiety, depression, ADHD, etc.
  • Complex physical health need, such as gastrointestinal conditions, neurologic conditions, developmental disorder, metabolic condition (like obesity or malnutrition), etc. 
  • Pregnancy, ADLs, Emergency Department utilization (2+ visits in the last six months)

Please note: not all health conditions qualify you for this specific program.

We will redetermine eligibility every 6 months. If you qualify, you can continue the program.
